Introduction to Rhinoplasty

As the central focal point of our face, our noses are metaphorically the “key” to all of our facial features. The right proportion of a nose can enhance every other important facial feature and create an everlasting first impression. However, when these various features are not in harmony with each other, a nose will draw the most attention and might appear narrow, wide, flat, or even bumpy. This is when most people might turn to the ever-popular “nose job” or Rhinoplasty.

What is Rhinoplasty?

More commonly referred to as a “nose job”, cosmetic Rhinoplasty includes various techniques aimed at improving the aesthetic and functional traits of a nose. The procedure can be performed as a closed rhinoplasty or an open rhinoplasty.

Patients who seek more minor changes in their nose, like adjusting the nasal tip, might be looking at a closed rhinoplasty (i.e. incisions limited to the inside of the nostrils), but when patients need larger or more complicated changes, such as altering the bridge and/or deeper alterations of the cartilage, an open rhinoplasty is used.

Most of the time, Caucasian noses require reduction rhinoplasty, with the nasal bones reduced in height and tip tripod structure supported with strong cartilage pieces harvested from self. On the other hand, Asian noses are more delicate and more complicated in anatomy, which requires more specific knowledge and better maneuvers to increase the dorsum height and tip support, to obtain satisfactory results. 

Using either technique during your nose reshaping, the skin is elevated, and the nasal bones and cartilage are altered.

Revision Rhinoplasty (Corrective Surgery)

Corrective surgery for a failed nose job – Dr. Yu treats all kinds of failed nose jobs, especially after implant surgeries. An in-person consultation with Dr. Yu would greatly help in designing the salvaging plans.

Combination Nose Surgery

It is becoming a new trend for Asian patients to go through a comprehensive combination surgery to correct their nose shape. Dr. Yu typically chooses to use an implant for simple Asian rhinoplasties. However, when the anatomy is difficult, or the deformity more severe, Dr. Yu will perform the rhinoplasty with different techniques, such as closed rhinoplasty (through the inside of the nose, or open rhinoplasty (via small incision across the columella. Either AlloDerm (a dermal filler made of collagen, elastin, and laminin), the patient’s own ear or rib cartilage, or other relevant materials may be chosen. Any or all of the above-mentioned steps/approaches could be selected and carried out for the desired nose shape.

Canine Fossa (Base of Nose) Augmentation

Canine fossa depression (base of the nose deficiency/depression) can be treated with fat grafting if minor, or with self-cartilage grafting and or AllodDerm if moderate to severe. A combination of all three approaches could be used for even better and much more coordinated aesthetic correction.

Many people, especially in the Asian population, have an underdeveloped Maxilla, causing the lateral and three-quarter views of the face to appear flat or even sunken, particularly in the mid-portion. The associated problems could include the under-supported base of the nose (Tripod structure) and deep furrow at the nasolabial folds. The treatment would include fat grafting (for mild to modest conditions) with either own ear cartilage fragments or AlloDerm, or a combination of all the above (for severe defects). 

For people against surgery, fillers could be used to correct mild to severe defects, with the understanding that the fillers may need to be repeated as required. Regardless of which procedure is chosen, the aesthetic effect is very profound and satisfying.

Columella Veiling 

Columella veiling/columella show is very easy to correct, provided that the surgeon knows how to correct it. Our doctor utilizes an extremely minimally invasive approach to augment the columella, for either its thickness or its vertical supporting strength, mostly via an intra-oral approach. The result is no visible scarring from anywhere on the face or nose and the recovery is super-fast!

Nasal Alar Reduction

Nasal alar reduction works to shrink the size of the nostrils. Dr. Yu uses an internal approach, by making incisions at the lateral aspect of the nasal sill to remove a wedge of tissues, including the corresponding nasal floor. After the mucosa is trimmed, the floor tissue can be turned down to augment the canine fossa (base of the nose) right then and there. At the same time, the nasal alar is brought in to cinch down on the size of the nostril. The recovery is fast, and the scars are barely detectable. 

Rib Cartilage Nose Job

Due to its abundance, rib cartilages could be utilized to correct both nose bridge insufficiency and the tip/columella complex. However, at this point in time, the revision rate remains relatively high among all plastic surgeons.

Rhinoplasty First Step

What’s The First Step?

The first step to get a Rhinoplasty in Arcadia, Los Angeles, and the surrounding Pasadena area is to schedule an initial consultation. When you come in, Dr. Yu will ask about your medical history, perform a complete examination of your face and nose, and discuss the various techniques available so that you can arrive at the best match for you. He may have photos taken, as well as share past photos of patients, to arrive at your best aesthetic goals.

If you choose to pursue your surgery, Dr. Yu will create a surgical plan and you will speak with our Patient Coordinator for scheduling, pricing options, as well as any pre-operative instructions.

What to Expect on the Day of Rhinoplasty

On the day of your Rhinoplasty, Dr. Yu and his friendly staff will welcome you and summarize your surgical plan in case of any final questions and/or concerns. He will mark the target areas, and then administer you with anesthesia for your comfort. 

Once the anesthesia has taken effect, Dr. Yu will start with an incision based on the technique discussed during your consultation. Next, cartilage may be harvested from the septum, the ear, or even your ribs depending on what is required. Dr. Yu will then use grafts and other techniques to shape the nose and improve breathing if that is a concern. Finally, the incisions will be closed with sutures. 

In rare cases, the nasal bones will need to be fractured and repositioned for restructuring. A splint will then be placed over the nose. Sometimes, your provider will place internal splints, as well.

Rhinoplasty Recovery

You will be monitored in the recovery room until you wake up. Please be sure to have family or a friend to help drive you home. You will likely have some minor pain, swelling, and have some black-and-blue bruising around and under the eyes. Your provider will discharge you with cold compresses and pain medication to address this.

 In one week, your provider will see you to monitor your recovery and remove your splint and sutures. You should keep your head elevated and iced, as needed, to minimize swelling.

Swelling can take about four to six weeks to resolve. For best results, you will have to wait a year for the final setting of your nose as well as the resolution of your scars.
